Термин «альтернативные клинические противомикробные препараты для животноводства» включает широкий спектр доступных вариантов, включая продукты с разными химическими структурами и механизмами действия, различные целевые группы животных, разнообразные пути введения и ожидаемые эффекты. Помимо веществ, предназначенных для замены антибиотиков, существуют также различные стратегии, связанные с поддержанием условий биобезопасности, соответствующей среды выращивания (например, уменьшение плотности посадки животных) и сбалансированного питания [51]. Кроме того, в качестве стратегии снижения устойчивости к противомикробным препаратам проводится анализ возникновения определенных инфекционных заболеваний в данной области (эндемичных) или в стране, а также осуществляются соответствующие графики вакцинации и административные мероприятия, связанные с ликвидацией инфекционных заболеваний [20].

Некоторые альтернативы, например, пробиотики, пребиотики, эфирные масла, органические кислоты и кормовые ферменты известны и используются в течение многих лет [26]. В свою очередь, антимикробные пептиды, бактериофаги, молекулярные частицы, хищные бактерии, Cas9 и аналогичные продукты с целевым действием на определенные части генома, связанного с бактериальной вирулентностью, все еще требуют дальнейших исследований, особенно с точки зрения их использования в животноводстве [58]. Их эффективность зависит от многих факторов, таких как способ введения (пищевые болюсы, прямое нанесение на вымя или инъекция), возраст животного и цель введения.

Каждая группа веществ имеет свои преимущества и недостатки и может быть предназначена для профилактики или лечения определенной группы заболеваний или может действовать в более широком диапазоне [38].

Поскольку альтернативы противомикробной терапии составляют огромную и разнообразную группу, очень проблематично дать исчерпывающий обзор всех доступных веществ или веществ, рассматриваемых как потенциальная альтернатива. Более того, часто следует рассматривать не применение только одного альтернативного варианта, а скорее комбинацию, по крайней мере, двух различных веществ или даже комбинацию различных стратегий действия и различных групп препаратов.

Выводы

Несмотря на внедрение большого количества стратегий противодействия резистентности, включая маркетинг альтернатив для замены антибиотиков, действия, последовательно предпринимаемые для ускорения глобальной тенденции к снижению лекарственной устойчивости, не кажутся успешными. Хотя наибольшая доля в использовании антибиотиков приходится на развивающиеся страны (и ожидается их дальнейший рост до 2030 года), производство продуктов питания животного происхождения в высокоразвитых странах остается на том же уровне, а производители по-прежнему внедряют и развивают системы производства, зависимые от антибиотиков.

Использование антибиотиков значительно снижает экономический риск производства, поскольку снижаются затраты на обслуживание и лечение животных. В большинстве стран неограниченный доступ к дешевому источнику протеина по-прежнему является главным приоритетом, а не проблема растущей лекарственной устойчивости, которая является абстрактным понятием для большинства населения. Дополнительным фактором, способствующим провалу глобальных действий по борьбе с лекарственной устойчивостью, являются неэффективные информационные кампании в СМИ, посвященные использованию антибиотиков; следовательно, общественное мнение не оказывает эффективного давления на деятельность правительств штатов.

Таким образом, согласование современной политики, экономики и охраны общественного здоровья должно стать императивом, в котором важную роль должны сыграть мероприятия, направленные на разработку, внедрение и маркетинг альтернативных препаратов антибиотикам или пропаганда рационального и обоснованного использования существующих антибиотиков.

Важно сохранять преобладание над антибиотикорезистентностью микроорганизмов, успевая разрабатывать и внедрять новые альтернативы, поскольку развитие трудно поддающихся лечению инфекций происходит очень стремительно.
