Благодаря своим уникальным характеристикам ФК/ФД (Фармакокинетики/Фармакодинамики) рабепразол является эффективным вариантом лечения кислотозависимых заболеваний. Он обеспечивает быстрое и стойкое устранение симптоматики, что может улучшить приверженность терапии у пациентов, а также помочь в соблюдении режима лечения. Благодаря быстрому развитию действия рабепразол может быть идеальным кандидатом для поддерживающей терапии «по требованию», более того, тот факт, что эффективность рабепразола не зависит от ИМТ, а также низкий потенциал взаимодействия между препаратами может быть обоснованием для его выбора у сложных больных (то есть пациентов с избыточным весом/ожирением, пожилых больных, пациентов с полипрагмазией).
Рабепразол является ИПП второго поколения с уникальными свойствами ФК и ФД он обладает максимальным значением рКа 1, наиболее быстрым развитием действия и наиболее мощным эффектом после первого приема. На него также в меньшей степени влияет статус генотипа CYP2C19 пациента.
По данным недавнего метаанализа в настоящее время отсутствуют прямые сравнительные исследования рабепразола и эзомепразола, которые показали превосходство в заживлении эзофагита над омепразолом, лансопразолом и пантопра-золом [42,81]. Тем не менее, в отдельных подгруппах больных, например, у пациентов с избыточным весом/ожирением наблюдается не только сохранение клинической эффективности рабепразола, но в этой популяции, по-видимому, наблюдается более выражен эффект препарата. У них наблюдается более быстрый ответ на рабепразол (при оценке по облегчению симптоматики) по сравнению с омепразолом [52]. Несмотря на сложность сравнения результатов, полученных при применении других ИПП в различных исследованиях вследствие различий в популяциях больных и дизайне экспериментов, другие исследования показали, что на активность прочих ИПП (например, эзомепразола) негативно влияет ИМТ [82,83]. Рабепразол обладает прекрасными показателями безопасности и переносимости, о чем свидетельствуют результаты непрерывного пятилетнего применения [62,63]. Кроме того, у данного препарата крайне низкий потенциал взаимодействия с другими лекарственными средствами [18, 19, 71-76].
Перспектива в отношении следующих 5 лет
Благодаря своему мощному и длительному эффекту в отношении снижения кислотности ИПП считаются препаратами выбора при кислотозависимых заболеваниях. Тем не менее, необходим ряд улучшений терапии, в особенности с целью обеспечения более качественного контроля ночных значений рН, достижения стойкого и надежного ответа у всех пациентов с ГЭРБ, включая больных НЭРБ, а также улучшение заживления при эрозивном эзофагите более высокой степени тяжести. Для разрешения этих актуальных терапевтических проблем в настоящее время проходят клинические исследования новые лекарственные формы ИПП, новые ИПП длительного действия, например, тенатопразол и новые антисекреторные препараты, например, так называемые обратимые ингибиторы протонного насоса (конкурирующие с калием кислотные блокаторы). Обоснование разработки этих препаратов заключается в том, что длительная супрессия кислотности может обеспечить улучшение скорости заживления при тяжелом эзофагите (вследствие более длительного снижения воздействия кислоты на пищевод), а также уменьшить частоту и длительность ночных эпизодов рефлюкса кислоты, что обеспечит лучший контроль ночной симптоматики без дополнительных препаратов (антагонисты Н2-гистаминовых рецепторов). Тем не менее, ни один из перечисленных выше препаратов по различным причинам не поступил в продажу, и, вероятно, этого не произойдет в будущем. Последний появившийся на рынке США ИПП декслансопразол с модифицированным высвобождением (разработан с целью обеспечения профиля ФК с двумя пиками, что продлевает кривую зависимости концентрации в плазме от времени для данного препарата) отсутствует в странах Европы. Таким образом, в следующие 5 лет предполагается наличие современных ИПП без дальнейшего прогресса.
