бизма, спастических паралича при ДЦП и нейрогенной гиперактивности мочевого пузыря у пациентов с менингомиелоцеле.

Недавно опубликованное проспективное исследование, посвящённое примене- нию Ботулотоксина-А в лечении гиперактивного мочевого пузыря у детей, пока-

зало улучшение при долгосрочном наблюдении в 70%. Несмотря на хорошие результаты, Ботулотоксин-А до сих пор не одобрен к применению у детей. Хорошие результаты при лечении дисфункций мочеиспускания были также получены в проспективном исследовании в Белграде. Как бы там ни было, мы советуем дождаться более обоснованных исследований применения Ботулотоксина-А в лечении дисфункций мочеиспускания, прежде чем применять его в широкой практике.

4. Заключение

Нарушения нижних мочевых путей у детей довольно частое явление. Выше был описан примерный протокол обследования и лечения. Как бы там ни было, нераскрытыми остаются вопросы: Какова этиология данных состояний? Является ли

лечение этиологическим? На сегодня чётких ответов на эти вопросы нет. Кроме

того, мы не знаем, есть ли связь между нарушениями мочеиспускания в детском возрасте и у взрослых. Действительно ли мы излечиваем детей с дисфункциями мочеиспускания, или лишь добиваемся временного улучшения с возможным возвращением симптомов во взрослом состоянии.

Данные литературы в этом вопросе довольно скудные, а объективность низкая. Исходя из имеющихся данных, можно сделать заключение, что связь между нарушениями мочеиспускания в детском возрасте и во взрослом состоянии имеется. Несмотря на временное устранение симптомов, некоторые дети будут иметь проблемы нижних мочевых путей, повзрослев.

У урологов, занимающихся проблемами недержания мочи у взрослых, в голове должна остаться мысль, что если данный пациент в детстве страдал недержанием мочи, то это усугубляет ситуацию и требует дополнительного внимания.

Детским же урологам следует предупреждать «вылеченных» детей, что имеется

определённый риск возникновения недержания мочи во взрослом состоянии.

Ключевые позиции

•Нейрогенный мочевой пузырь в большинстве случаев связан с врождёнными

пороками, реже причиной выступают опухоли, травмы и инфекция.

•Визуализирующие и уродинамические обследования начинаются как можно

раньше и повторяются в течение всего периода наблюдения.

•Необходимо сосредоточиться на сохранении функции почек и мочевого пузы- ря, коррекцию недержания мочи можно проводить позднее.

•Периодическая чистая катетеризация (ПЧК) и холинолитические препараты

должны быть включены в лечение детей с менингомиелоцеле с младенчества.

1. Введение

Нейрогенныймочевойпузырь(НМП)вбольшинствеслучаевсвязансврождён- ными пороками, реже причиной выступают опухоли, травмы и инфекция. Наиболее частой причиной возникновения НМП является спинальный дизрафизм, порок развития эмбриональной нервной трубки и дужек позвонков, наиболее часто возникающий в дистальной части спинного мозга. Прогноз в отношении функции почек и мочевого пузыря у таких пациентов колоссально улучшился за последние полвека, этим он обязан новым стандартам обследования и лечения, среди которых, несомненно, самым главным является введение ПЧК.

2.Причины НМП у детей

2.1.Нейроспинальный дизрафизм

Миеломенингоцеле (ММЦ) – термин, обозначающий протрузию твёрдой мозговой

оболочки и элементов спинного мозга через дефект дужек позвоночника, вызванный нарушением обрастания тканями мезодермы формирующегося спинного мозга. Также в процесс могут включаться элементы жировой ткани, так называемое, липомиеломе-

нингоцеле. ММЦ на сегодняшний день встречается с частотой лишь примерно 2-3 на

10000 новорождённых детей благодаря пренатальной диагностике и своевременному прерыванию соответствующих беременностей. Приём фолиевой кислоты во время бе- ременности также помог снизить частоту заболевания. Заболевание может поражать все уровни спинного мозга, но наиболее часто поражается пояснично-крестцовый отдел (поясничный 26%, крестцовый 20%, пояснично-крестцовый 47%). Уровень поражения

даёт немного информации о прогнозе степени поражения мочевого пузыря и нижних конечностей. Гидроцефалия, вызванная окклюзией водопровода четвёртого желудочка
