7.1. Визуализация
Ультразвуковое исследование, микционная цистоуретрография и DMSAсканирование - это три главные опции визуализации для обследования детей с ИМВП. В соответствии с недавними исследованиями, обследование с использованием визуализации, разработанной для детей с первым эпизодом неосложненной ИМВП, может не изменить тактику ведения больного.
Другие исследования утверждают, что однократное урологическое расстройство встречается у меньшинства детей с ИМВП. Поэтому после двух эпизодов ИМВП у девочек и одного эпизода у мальчиков, визуализация должна быть выполнена в обязательном порядке.
Использование всех трех техник визуализации почки - нереалистично и не практично. «Золотым стандартом» визуализации должен быть безболезненный, неинвазивный, безопасный, достаточно надежный для определения любой структурной аномалии, рентабельный метод. В настоящий момент не существует радиологической
техники, удовлетворяющей всем этим требованиям.
Ультразвуковое исследование мочевых путей используется для диагностики врожденных аномалий развития, обструкции и рубцевания почки; микционная цистоуретрография – для идентификации пузырно-мочеточникового рефлюкса, а ДМСА-сканирование - для определения рубцевания почки.
7.1.1. Ультразвуковое сканирование
•Это безопасная, быстрая, рентабельная методика с высокой точностью опре- деления анатомии и размеров почечной паренхимы и собирательной системы.
•С другой стороны - это субъективный метод, не предоставляющий информа- ции по почечной функции.
•Одного УЗИ недостаточно для исключения пузырно-мочеточникового реф- люкса. Ребенок с высоким рефлюксом по данным микционной цистоуретрогра-
фии может иметь абсолютно нормальные данные сонографии.
•УЗИ мочевого пузыря может быть использовано для определения остаточно-
го объема мочи после мочеиспускания

7.1.2. Микционная цистоуретрография (МЦУГ)
•Соблюдение стерильности мочи обязательно для проведения МЦУГ
•Необходима при ИМВП у детей младше года

•До 23% детей с фебрильной температурой при ИМВП и нормальной УЗ-
картиной могут иметь пузырно-мочеточниковый рефлюкс на МЦУГ
•Недостатки включают: риск инфекции, инвазивность метода, воздействие
радиации
•Флюороскопическая МЦУГ может быть использована вместо традиционной
МЦУГ для минимизации дозы радиации
•Время проведения МЦУГ при обследовании ребенка с ИМВП остается про- тиворечивым. В некоторых исследованиях подчеркнуто, что МЦУГ должна быть выполнена через 4-6 недель после острой фазы инфекции.
•В соответствии с некоторыми недавними исследованиями, МЦУГ может быть
выполнена сразу же после элиминации инфекции у ребенка.
7.1.3. ДМСА-сканирование (димеркаптосукциновая кислота)
•ДМСА-сканирование по праву остается «золотым стандартом» в прогнози- ровании рубцевания почки. Специфичность и чувствительность при почечном сканировании составляют 100 и 80%, соответственно.
•Эта методика показательна в определении функционирующей массы почки и
рубцевания коркового слоя почки.
•Звездообразные дефекты в ренальной паренхиме могут указывать на острый
эпизод пиелонефрита
•Фокальныедефектывкорепочкичастоуказываютнахроническоеповреждение.
•ДМСА-сканирование может помочь в прогнозировании исхода у ребенка с
инфекцией МВП.
